Submitting a report is the best way to help us all be safe. These are directed to specific employees who can respond, reach out, or provide resources based on the issue, need, complaint, or concern.

There are multiple reporting options at Ë¿¹ÏÊÓƵ Tech: Anonymous Reporting, Confidential Reporting with the Title IX office, and formal reporting or concern sharing forms. The  (anonymous or non-anonymous) is routed directly to the Title IX Coordinator. This form CAN be used even if you are unsure what you may have experienced and want to talk to someone about it.  (non-anonymous) is available in TECHweb to make a formal report to the Dean of Students for a variety concerns. Student Affairs Report Form (anonymous) is available for you to print and submit this paper form which helps Ë¿¹ÏÊÓƵ Tech gather information regarding the incidence of sexual and dating violence on and around our campus so that we may better educate and protect our community members.

Anyone can also make a report to local law enforcement by calling 911 or visiting .

  • Anonymous or Non-anonymous: This form is routed directly to the Title IX Coordinator and can be completed to report sexual misconduct, sexual violence, harassment, domestic/ dating/ intimate partner violence, discrimination, behavior that limited one's access to education on the basis of sex that you have experienced or observed. This form CAN be used even if you are unsure what you may have experienced and want to talk to someone about it.
  • Use this form to make a formal report concerning academic integrity, arson/fire, assault, dating violence, discrimination, domestic violence, drugs, harassment, hate crime, bias incident, sexual assault, stalking, theft, threatening behavior, vandalism, weapons, or concerns of another nature.
  • Use this form to formally share about any concerns that you have about yourself or a student who is struggling academically, personally, financially, disruptive behavior, physical needs, or another concern.

  • Print and submit this paper form which helps Ë¿¹ÏÊÓƵ Tech gather information regarding the incidence of sexual and dating violence on and around our campus so that we may better educate and protect our community members.

Frequently Asked Questions

  • Ë¿¹ÏÊÓƵ Tech (OT) will help to ensure that the person who experiences sexual violence is safe, including while the investigation is ongoing. OT will take interim measures as needed.
  • OT will respond promptly and effectively to reports of sexual violence.
  • If OT knows (or should reasonably know) about possible sexual violence, it will promptly conduct a neutral investigation to determine what occurred and then take appropriate steps to resolve the situation.
  • The person who experienced sexual violence will be informed of their options to notify law enforcement. If there is a criminal investigation, OT will continue its efforts to resolve reports promptly and effectively.
  • Complainant and Respondent will have timely access to information that will be used in a student conduct hearing.
  • Complainant and Respondent will both have the opportunity to have an advisor present, present evidence, and have witnesses speak during disciplinary proceedings.
  • Both parties will be informed in writing of the outcome of any OT disciplinary proceeding.
  • Both parties have a right to appeal the decision.
